Application For Assistance
We are here to help adolescents with Scoliosis find a place, seek treatment, gain support and love, and aspire to become who they want to be. The Straight Paths Fund receives many applicants and we are motivated, dedicated, and obligated to assuring that each applicant receives a response and are equal within the consideration process of delegation of funds for surgeries.
To apply for help from The Straight Paths Fund, we require that you verify some simple information so that we can better understand your needs and offer help accordingly. Begin by filling in the information below. Please understand that as soon as your application is processed, we will notify you and contact you as soon as possible. This may take several business days due to the amount of applications we receive.
Due to both legal reasons and the amount of applications we receive, we require that the child being applied for is a direct family member and has given consent to the application. Applications may not be processed for a child unless you are the legal guardian of said child. If a section is not applicable please insert N/A. If you have any questions, feel free to Contact Us.
Applicants must meet the following criteria to be considered for a grant:
Applicant must be a US citizen 21 years of age or younger at the time of application.
Applicant must be seeking financial help only for his/her Scoliosis surgery, we do not offer financial help for other treatments at this time.
Applicant must have a diagnosis of Scoliosis with curves measuring over 40 degrees.
Applicant must have a Social Security Number issued by the Social Security Administration. TIN numbers are not accepted.
Family must not exceed maximum eligible family income as documented on IRS Tax Form 1040.
$55,000 or less for a family of 2
$85,000 or less for a family of 3
$115,000 or less for a family of 4
$145,000 or less for a family of 5 or more
Primary coverage for the adolescents must be by a commercial health plan, either through an employer or individually purchased. Secondary insurance through Medicaid or CHIP is permissible.
Non-US citizens - please ask a local nonprofit or church to submit an application form on your behalf. International grant funds are typically disbursed through registered charitable organizations.
For discounted travel - lodging and meals for families may be available through a Ronald McDonald House located nearby, at little to no cost.
